Before your dog's first grooming appointment, we ask every pet parent to complete our Customer & Pet Information Form. A separate form must be completed for each pet, even if multiple pets are from the same household. This form is required for all grooming services, including paid, discounted, complimentary, and training appointments.

You may notice that we ask quite a few questions—and there's a good reason! Every dog is different. Information about your pet's health, vaccinations, medications, allergies, grooming history, temperament, behavior, sensitivities, and previous experiences helps us understand their individual needs before grooming begins.

Knowing this information allows us to prepare, make appropriate accommodations, reduce unnecessary stress, and provide the safest and most comfortable grooming experience possible for each individual pet.

The form also includes our important grooming policies, safety guidelines, emergency authorization, owner consent, and liability waiver, so you know what to expect before services begin.

Please complete ONE form for EACH pet receiving grooming services.
